Open and closed forms of pyonephrosis are distinguished. In the first case, products of purulent inflammation enter the urine, being excreted along with it through the ureter. In this case, the urine analysis shows an increased content of leukocytes. In the closed form, the focus of inflammation is surrounded by a dense capsule made of connective tissue elements, which prevents the outflow of urine and retains pus. Since urine is only excreted from the unaffected kidney, laboratory tests cannot detect inflammation. In the absence of timely adequate treatment, the closed form can transform into an open one.

In the first days of the disease, the patient's condition is satisfactory, with aching pains in the lumbar region and a subfebrile temperature. As the pathology progresses, the condition worsens: the temperature reaches 40-41℃, symptoms of severe general intoxication appear, and intense pain occurs, up to renal colic. One of the most characteristic symptoms of pyonephrosis is cloudy urine due to the presence of tissue breakdown products. Cloudy urine is observed in the open form of the disease, which is also characterized by weak expression or absence of signs of intoxication.

In cases of kidney damage and absence of signs of ureter involvement, complete removal of the affected kidney — nephrectomy — is indicated. The operation is performed laparoscopically or by open method. The advantages of laparoscopy include a low probability of damage to healthy tissues and the development of complications, as well as a short postoperative period. If the initial cause of the purulent inflammation is stenosis of the ureteral lumen, nephroureterectomy — removal of the kidney along with the ureter — is performed. This is also done laparoscopically or by open method.

The preparation period for nephrectomy takes, on average, a week and requires hospitalization of the patient. In addition to comprehensive laboratory and instrumental examinations, consultations with a nephrologist, therapist, cardiologist, and several other narrow-profile specialists are necessary. Urgent surgery is required in cases of acute closed form of the disease, as there is a high probability of capsule damage and pus entering the abdominal cavity, leading to peritonitis and sepsis.

In cases of severe patient condition, presence of serious comorbidities, impaired normal function of the unaffected kidney, or diagnosis of bilateral pyonephrosis, surgical intervention is performed in two stages. Initially, a nephrostomy is placed (an artificial channel between the kidney and the skin surface for urine drainage), allowing for the drainage of the focus of purulent inflammation. Simultaneously, antibiotic therapy, detoxification, and plasmapheresis are prescribed to the patient. After stabilization of the condition, nephrectomy is performed.

Diagnostic Methods for the Disease
