This type of diagnostics allows for a detailed examination of the mucosal condition, identification of anatomical anomalies, and a range of diseases. Typically, in Israeli clinics, this type of diagnostics is performed for the following indications.

Procedure Conduct

The procedure is conducted using a special endoscopic device called a hysteroscope. It is a thin metal tube, at the distal end of which there is a camera and a miniature light source. Data from the camera is transmitted via a fiber optic cable to an eyepiece. Observing through the eyepiece the internal structures of the uterus, the physician conducts diagnostics or therapeutic procedures.

Gynecologists in Israeli medical centers prefer to use more modern models of endoscopic equipment — ultra-thin flexible fibrous hysteroscopes. These are advanced systems that provide very high image quality and allow recording the image from the eyepiece to a computer for subsequent processing and analysis. The diameter of the inserted tube of such devices barely exceeds three millimeters, and its distal end can bend at an angle of more than 120 degrees, which broadens the capabilities of the device even further.

Conducting the ProcedureThe best time for conducting diagnostics is the period following the end of menstruation, when the thickness of the endometrial layer is relatively small. Depending on the purpose of the procedure, local or general anesthesia is used. Sometimes anesthesia is not required. Some patients may undergo cervical dilation beforehand, although the diameter of modern hysteroscopes is so small that in most cases dilation is unnecessary.

During the examination, the hysteroscope is inserted through the vagina into the uterine cavity, which is inflated with carbon dioxide or filled with a special saline solution. This is standard practice for conducting any cavity endoscopic procedures. It improves the view of the internal structures of the examined cavity. Carbon dioxide is used if the procedure is aimed at diagnostics. Electrolyte solutions are used in both diagnostic and surgical hysteroscopy. If solutions are used, electro-surgical operations can be performed using bipolar equipment.

Observing through the eyepiece, the physician examines the condition of the inner surface of the uterus. If anomalies are detected that can be treated immediately, the physician performs the necessary manipulations using a surgical hysteroscope. Surgical intervention can be performed using standard micro-surgical morcellators, lasers, or mono- or bipolar electro-surgical instruments. Typical surgical procedures include endometrial ablation, submucosal resection of fibrous formations, and polypectomy.
