What is pneumonectomy?

This is a surgical operation to remove an entire lung. It is performed when severe lesions of this organ are detected in the patient.

Indications for pneumonectomy include:

  • empyema of the pleura;
  • cavernous-cirrhotic tuberculosis of the lungs;
  • fibrous-cavernous tuberculosis of the lung with multiple tuberculomas or the presence of cavities in several lobes.
  • central peribronchial lung cancer with a malignant process and involvement of the main bronchus.

Thoracotomy via lateral access is performed in the area of the 5th intercostal space, anterior access – in the area of the 4-5 intercostal spaces, and posterior access – in the area of the 6th intercostal space.

After the lung is isolated, the pulmonary ligament is ligated and dissected. The mediastinal pleura is dissected parallel to the phrenic nerve.

Then, during right-sided pneumonectomy, the right pulmonary artery is isolated and ligated, which is then sutured and transected. Similar actions are performed with the main right bronchus and the upper and lower pulmonary veins. After that, pleuralization (suturing) of the mediastinal pleura flaps is performed.

During left-sided pneumonectomy, the left pulmonary artery and veins are also isolated, sutured, and transected. The bronchus itself is isolated up to the tracheobronchial angle, which is then processed and transected. In this case, pleuralization of the stump of the left bronchus is not performed because it will go under the aortic arch into the mediastinum.

Pneumonectomy lasts about 3 hours, and the postoperative hospitalization period is 5 days.
