For locally advanced and metastatic forms, next-generation hormonal therapy (enzalutamide, abiraterone, apalutamide), targeted PARP inhibitors for BRCA mutations, and radioligand therapy ¹⁷⁷Lu-PSMA and ²²³Ra (Xofigo) for bone metastases are used.

A key feature of the center is the multidisciplinary Tumor Board: a urologic oncologist, radiation therapist, medical oncologist, radiologist, and pathologist jointly review each case and propose a protocol that aligns with international guidelines from EAU and NCCN. For patients with early-stage and low-risk disease, Ichilov offers an active surveillance program — instead of aggressive treatment, structured monitoring of PSA, MRI, and repeat biopsies is conducted, allowing for the postponement or complete avoidance of intervention without compromising prognosis.
