Prostate cancer is one of the most common types of cancer in men, but with timely diagnosis, it is among the most manageable oncological diseases. At the Ichilov (Sourasky) Medical Center in Tel Aviv, the treatment of prostate cancer is structured as a single pathway: from targeted diagnostics to personalized therapy and monitoring.
The diagnostic unit relies on high-resolution multiparametric MRI (mpMRI) and fusion-guided biopsy (MRI/Ultrasound) — this allows for targeted investigation of suspicious areas and, in some cases, avoids "blind" biopsy. For staging, PSMA-PET/CT is used, which detects microscopic foci and metastases that are invisible on standard images and directly influences the choice of strategy.
In the treatment of prostate cancer at Ichilov, the entire modern arsenal is applied: robot-assisted radical prostatectomy using the da Vinci system with preservation of neurovascular bundles (reducing the risk of incontinence and erectile dysfunction), stereotactic body radiotherapy (SBRT), and brachytherapy — including accelerated radiation protocols that allow completion of the course in 1–2 weeks. For locally advanced and metastatic forms, next-generation hormonal therapy (enzalutamide, abiraterone, apalutamide), targeted PARP inhibitors for BRCA mutations, and radioligand therapy ¹⁷⁷Lu-PSMA and ²²³Ra (Xofigo) for bone metastases are used.
A key feature of the center is the multidisciplinary Tumor Board: a urologic oncologist, radiation therapist, medical oncologist, radiologist, and pathologist jointly review each case and propose a protocol that aligns with international guidelines from EAU and NCCN. For patients with early-stage and low-risk disease, Ichilov offers an active surveillance program — instead of aggressive treatment, structured monitoring of PSA, MRI, and repeat biopsies is conducted, allowing for the postponement or complete avoidance of intervention without compromising prognosis.
