Sourasky Oncology Center: multidisciplinary teams, organ-preserving surgery, and targeted therapy
At the state medical center Ichilov (Sourasky) in Tel Aviv, breast cancer treatment is conducted at the Sourasky Institute of Oncology — the largest in Israel. Here, patients are simultaneously managed by a multidisciplinary team: oncologist-mammologist, surgeon, radiotherapist, pathologist, geneticist, reconstructive plastic surgeon, and psycho-oncologist. Diagnosis relies on digital 3D mammography (tomosynthesis), ultrasound with elastography, MRI of the breasts with contrast, image-guided core biopsy, and molecular genetic profiling of the tumor. To assess the risk of recurrence in hormone-dependent tumors, tests such as Oncotype DX and Prosigna are used, and for hereditary forms, analysis of BRCA1/BRCA2 and expanded genetic panels. This allows for the avoidance of excessive chemotherapy where it does not improve prognosis.
Treatment includes breast-conserving surgeries (lumpectomy, oncoplastic resection), skin-sparing and nipple-sparing mastectomy, immediate reconstruction with an expander/implant or using the patient's own tissues (DIEP flap), intraoperative radiation therapy, modern linear and stereotactic radiotherapy. The medication arsenal includes targeted therapies (trastuzumab, pertuzumab, T-DM1, trastuzumab-deruxtecan), CDK4/6 inhibitors, PARP inhibitors, immunotherapy for triple-negative cancer, as well as hormonal therapy with side effect prevention.
