Breast cancer is the most common malignancy in women. The Ichilov Breast Centre operates within the Institute of Oncology and brings together a multidisciplinary team: breast specialists, oncologic surgeons, medical and radiation oncologists, and reconstructive plastic surgeons.
An accurate diagnosis is the foundation
The standard diagnostic package includes:
- Digital 3D mammography and breast ultrasound;
- Contrast-enhanced breast MRI to assess disease extent;
- Ultrasound-guided core biopsy with histology and immunohistochemistry (HER2/neu, ER, PR, Ki67);
- The Oncotype DX test — assesses recurrence risk and the need for chemotherapy: ~70% of patients can safely avoid chemo;
- PET-CT or PET-MRI if metastatic disease is suspected.
Breast-conserving surgery
At Ichilov, 90% of breast cancer operations are breast-conserving. We perform:
- Lumpectomy with sentinel lymph node biopsy;
- Oncoplastic surgery — tumour removal with simultaneous reshaping and symmetry correction;
- Radical mastectomy with immediate or delayed reconstruction (implants, DIEP flap);
- Cryoablation — image-guided freezing of small tumours without an incision.
Systemic therapy
- Adjuvant and neoadjuvant chemotherapy per NCCN and ESMO guidelines;
- Anti-HER2 therapy (trastuzumab, pertuzumab, T-DM1, T-DXd) for HER2-positive disease;
- Hormone therapy and CDK4/6 inhibitors (palbociclib, ribociclib) for hormone-positive disease;
- Immunotherapy (pembrolizumab) for triple-negative cancer with PD-L1 expression.
Radiation therapy
Used after breast-conserving surgery and in HER2-positive disease with node involvement. Ichilov is equipped with Linac MRI UNITY and ETHOS accelerators supporting adaptive planning. An alternative for selected early-stage patients is single-session intraoperative radiation therapy (IORT).
