Conservative therapy is indicated in the presence of a small hematoma with stopped bleeding. The patient is prescribed a range of medications that promote the resorption of the hematoma, antispasmodics, diuretics, anti-inflammatory drugs, coagulants, etc. If conservative therapy is conducted, the patient is under constant observation. Any deterioration in condition is an indication for emergency surgery.
Surgical treatment is performed for brain decompression and stopping bleeding from the damaged vessel. If necessary, a craniotomy may be performed. During the trepanation of the skull, a fragment of the skull bone is removed, the blood clot is extracted, the cavities are washed, and the damaged vessel is sutured or embolized.
If there is stopped bleeding but the hematoma needs to be removed, a minimally invasive surgery may be performed. Access to the hematoma will be through burr holes in the skull. Through the holes, the hematoma is removed using a micro-aspirator or ultrasound aspirator.
Patients with an intracranial hematoma require rehabilitation. Since there was compression of the brain area, it is necessary to restore the functions that were affected: speech, movement, memory, etc. Patients at the rehabilitation center are prescribed a comprehensive therapy that helps fully restore all functions. The program includes: physiotherapy, reflex therapy, training on simulators, therapeutic exercises, massage, and much more. Psychologists and psychoneurologists work with the patients.
