Types of Surgical Treatment for Sinusitis at Top Ihilov Medical Center
Sinusitis is an inflammatory process of the maxillary sinus, accompanied by all symptoms of inflammation, difficulty in nasal breathing, and headache. Usually, treatment for sinusitis begins with conservative measures, but if there is a complicated disease, an advanced form, and conservative treatment shows no dynamics, surgical intervention is resorted to.
Surgical treatment of sinusitis is indicated for any forms of the pathology: alternative, proliferative, and mixed. The intervention can be performed via both endonasal and extranasal access.
Endonasal access to the focus of pathology involves the use of reliable endoscopic and microsurgical equipment. All surgical manipulations are performed through the nasal cavities, leaving no postoperative scar. However, this operation is not always possible due to the anatomical features of individual patients. In cases where endonasal surgery is contraindicated, surgical intervention is performed using Caldwell-Luc or Denker methods.
At Top Ihilov Medical Center, the Caldwell-Luc method is more frequently used. This extranasal operation provides excellent access to the focus of pathology, is completely safe, and painless.
The intervention involves anesthesia: general or local. It lasts 2 – 3 hours. During the operation, the maxillary sinus is opened through the anterior wall, and the damaged mucosa, polyps, necrotic and purulent masses are removed. The second stage of the operation involves opening the ethmoid bone cell, removing all altered disease tissues localized in the upper medial corner of the maxillary sinus. The operation requires the patient to be hospitalized for 2 days.
To prevent the risk of bleeding from the surgical wound into the sinus cavity and the area of the ethmoid labyrinth, a gauze tampon or pneumatic tamponade, soaked in iodoform and antibacterial ointment, is placed. This procedure also prevents the development of inflammation.
Additionally, at Top Ihilov Medical Center, plastic closure of the perforation hole is performed after perforated odontogenic sinusitis. The plastic closure is carried out by moving tissue from the vestibule of the nasopharynx or palate.
