Liver cancer is one of the most complex oncological diseases: the tumor often develops against the background of cirrhosis or chronic hepatitis, so treatment requires the simultaneous efforts of oncologists, hepatologists, transplant surgeons, and interventional radiologists. Diagnosis includes MRI of the liver with hepatospecific contrast (Primovist), CT with triple-phase contrast, PET-CT, elastography, and ultrasound-guided biopsy; markers AFP, PIVKA-II, and molecular profiling of the tumor guide therapy selection.

The strategy at Ichilov is based on the BCLC stage. In early stages, liver resection (including laparoscopic and robot-assisted), radiofrequency and microwave ablation (RFA/MWA), and, when appropriate criteria are met, liver transplantation are performed. In intermediate stages, transarterial chemoembolization (TACE) and radioembolization (SIRT/Y-90) are used. In advanced stages, modern systemic therapy is introduced: combinations of immunotherapy (atezolizumab + bevacizumab, durvalumab + tremelimumab) and first- and second-line targeted therapies — sorafenib, lenvatinib, regorafenib, cabozantinib.

Can I get a second opinion on my records without travelling to Tel Aviv?

Yes, the "second opinion" programme is a remote service: histology, imaging and clinical data are re-reviewed and a written report by a leading oncologist of the Sourasky Institute of Oncology is issued within 5–10 working days. A trip is not required — all documents are accepted electronically.

How is follow-up organised after returning home?

The patient receives a detailed follow-up plan: schedule of control CT/MRI scans, laboratory tests and physical exams, plus recommendations for supportive therapy. An English- or Russian-speaking coordinator stays in touch; telemedicine consultations with the treating Ichilov physician are available, and adjuvant therapy can be adjusted without a repeat trip to Tel Aviv.
